We are considering signing up with Surety, but the main thing is to get a proper insurance certificate that indicates both fire and burglar monitoring and alarm. How can I see a sample of what Surety’s certificate will look like?
Also, is there a fee for it? How often can I request it?

To add to the above response, there is no fee. You can download your certificate at any time on our website when you have active service.
