I had reset my mobile phone (i.e. clean, fresh install), and therefore re-downloaded my apps, and reconnect everything (i.e. Bluetooth disarming etc.). Alarm.com saying there is a malfunction because it has communicated with the device in a while but its my first version of the device as my second version is connected and works fine.
How do I let alarm.com or my panel ‘forget’ my old phone so I don’t have to get this device malfunction message anymore?
There are two places this may be referring to. If you had the phone set up as a push device, go to Users, Push Devices, and you can delete the original instance of the phone.
This may also be referring to Geoservices if that phone was set up as a geo device previously. You can navigate on the Alarm.com website to Settings, Geoservices and click the phone’s name under Geo-Devices. Near the top will be a trash icon to delete that device.

So after deleting my push device, how do I add it again?
If you have two instances of your phone listed under push devices, you would only delete the original one which is in malfunction. The current one you say is working would be left alone.
As it does not appear this is related to a suretyDIY subscription I cannot confirm on the back-end, but from your description that is how it sounds.
Push devices are created simply by logging into the Alarm.com app on a phone and turning on Push notifications in the settings menu of the app.
I went to users it listed the users, and then it listed 1 push device. I deleted the push device. Went to my alarm.com app, enabled push notifications. Then I login into the web and it listed my push device in users. I then went back into my notifications, and re-assigned the push notifications.
However, it still shows up as ‘malfunction’ with the push device. Because the new and old push device have the same name, what I did was edit the name of newly added push device.
At this point, the malfunction message is still there and the name is from the old phone. Is it just a matter of the alarm.com sever just updating? Its been about 10 min but I can give it more time…
